  3. There is a command in the screen saver that creates an order
  4. form  to  be used to register the product.  It is  available
  5. under  the  "Order Form and Password" submenu  by  selecting
  6. "Create Order / Registration Form".  A sample of the created
  7. order  form is below.  Don't use it unless you can't  create
  8. the  order  form by using the application.  The  dialog  box
  9. displayed by the "Create Order / Registration Form"  command
  10. contains  a  Computer ID field.  The value from  that  field
  11. must  be present in this form (under Computer_ID) for us  to
  12. send  you  your  password.  See the Help  file  for  further
  13. information.
